  • Respite care: Temporary hospice services are available in a nursing home or other skilled care facility for up to 5 days.
  • Real-time support: Our telehospice service remotely monitors your symptoms each day. Hospice-trained telehealth nurses offer immediate support for physical and emotional concerns. Learn more about our telehospice program.
  • Veterans for Veterans Hospice Program: We offer service members a chance to volunteer in our program. We pair them with military veterans in our hospice care to provide comfort, understanding and a listening ear.
  • Grief support program: We provide support to families for 13 months after a loved one passes. Our help includes:

    • Individual grief support through phone calls and 1-on-1 meetings

    • Spring/Summer and Fall/Winter Memorial Services

Meet your hospice team

Our hospice professionals work together with you and your family to ensure that we meet your goals for end-of-life care. Your hospice team includes:

  • Doctor: oversees your medical care and advises the hospice team
  • Nurse: coordinates your care, focusing on pain and symptom management
  • Pharmacist: consults on any medication issues
  • Social worker: coordinates additional community resources for your family
  • Dietitian: specializes in end-of-life nutritional needs
  • Counselor: provides bereavement support and counseling
  • Chaplain: provides counseling and coordinates any spiritual support you need
  • Home health aide: assists with your day-to-day personal care

Starting home hospice care

More than 95% of the people who choose hospice receive their care at home. If you and your doctor decide to start home hospice, you can expect:

  • Assessment: A nurse evaluates your needs and your family’s needs for hospice care. Needs are then reassessed on an ongoing basis.
  • Hospice visits: Your case manager nurse will visit based on your condition, modifying the visit schedule as needs change.
  • 24/7 helpline: The helpline answers questions related to hospice care and arranges further care when needed.
  • Telehospice: Families using our telehospice program complete a daily questionnaire, allowing telehospice nurses to monitor symptoms between visits.
